The public form asks for contact details, organization context, statement volume, claim workflow, and a short message so ClaimAudio can respond to the request.
No confidential file details in public forms
Do not submit claim numbers, claimant names, medical details, policy details, recordings, transcripts, or other confidential claim material through the website form.
Pilot material is handled separately
Any sample statement review or pilot upload should use approved anonymized material and be governed by written data-handling terms before files are exchanged.
